Access the informed consent form
The informed consent form authorises the Office of Drug Control (Department of Health and Aged Care) to obtain information on an individual named in an application for the grant or variation of a licence under the Narcotic Drugs Act 1967.
If you have been nominated by an applicant to complete an informed consent form for their ODC application, you will receive an automated email that either:
- requests you to log into the HBS portal if you are already an existing portal user
- requests you to redeem a single-use invitation link to create a portal account.

Once you have logged in to the portal, you will need to navigate to the informed consent form and complete it. Follow the below process to do this.
- On the portal home page, click on Get started in the ‘Services’ tile.
- The ‘Services’ page will open. Click on Apply in the ‘Office of Drug Control’ tile.
- The ‘Office of Drug Control’ page will open displaying the available forms. Click Apply in the ‘Informed consent’ tile.
- The ‘Submissions’ page will open on the ‘Informed consent’ tab. This is where your informed consent form(s) will be.
- Click on Start in the ‘Action’ column to open the informed consent form.
- The form will open. Complete the details and click on Submit at the bottom of the page.
Note: You need to complete the form in one sitting. If you close the form without completing it, no details will be saved. You will still be able to open and start the form again.
